The Digital Home Care platform will connect healthcare centers with patients' homes through a two-way communication channel, enabling continuous monitoring of their condition without requiring them to visit healthcare facilities, as well as real-time communication. Its aim is to improve patients' quality of life by preventing hospital readmissions and frequent trips to healthcare centers.
 
The solution proposed by Indra and Telefónica includes two types of services: one for patients receiving hospital care at home; and another focused on the prevention, monitoring, and follow-up of illnesses—especially chronic conditions or those requiring rehabilitation—and promoting continuity of care at home.
 
This platform offers numerous features for both patients and healthcare professionals. These range from reconciling treatments to avoid interactions, incompatibilities, or duplications, to video consultations, sending self-care reminders, self-monitoring of biometric parameters (blood pressure, temperature, pulse, glucose, etc.), and providing nutrition, exercise, and rehabilitation plans.
 
More Accessible Services:
Patients will be able to access the platform through multiple devices—interactive digital TV, web, or mobile—and interact with healthcare professionals, regardless of their location, to complete procedures, consult their medical or medication history, configure notifications, establish health programs, or review educational materials about their condition.
 
For their part, professionals will be able to provide comprehensive patient follow-up and care from their homes, in a thorough manner and optimizing time by eliminating the need for travel and in-person visits.
 
The project is part of the InnovaSaúde healthcare innovation plan (www.sergas.es/h2050-innovasaude), which represents an investment of over €45 million, co-financed by ERDF funds within the R&D&I Operational Program for the benefit of businesses, Technological Fund 2007-2013.
